What is Form MD-9 (Manufacturing License)?
An MD-9 licence is the Central Licensing Authority’s permission under the Medical Devices Rules, 2017 to manufacture moderate-high (Class C) and high-risk (Class D) devices for sale or distribution at a specified site. Because of the higher risk, the application requires a full Device Master File and evidence of safety and performance, and may require clinical data.
Documents include MD-7 with fee, constitution documents, premises proof and plant layout, DMF under the Fourth Schedule, risk management, QMS documents such as ISO 13485, validation records, details of technical staff, equipment and testing facilities, clinical investigation or performance evaluation data or permission, labelling and IFU. Validity is perpetual subject to the retention fee as per CDSCO’s circular of 15 May 2024.
