What is Form MD-5 (Manufacturing License)?
An MD-5 licence is the State Licensing Authority’s permission under the Medical Devices Rules, 2017 to manufacture low-risk (Class A) and low-moderate-risk (Class B) devices for sale or distribution at a specified manufacturing site. The manufacturer must maintain the quality management and technical documentation required by the Rules and is subject to inspection.
Documents include constitution documents, premises ownership or lease proof and site plan, details of technical staff and qualifications, manufacturing process, machinery and testing facilities, Device Master File, labelling and IFU, and undertakings. Class A sterile or measuring devices also require a licence, while Class A non-sterile non-measuring devices are exempt.
